IncludeHealth provides an internationally awarded, inclusive, and integrated digital health platform transforming fitness, rehabilitation & wellness. As a TRL-9 technology commercialized in multiple sectors of care, the platform pairs HIPAA compliant cloud software with inclusive rehabilitation & wellness equipment to produce better outcomes with lower barriers and costs. This combination provides powerful next generation capabilities such as outcomes-based data collection, automated documentation, cloud collaboration and inclusive design With 6 years of R&D, $8M in venture capital, deployments throughout the continuum of care, and a successful Phase I SBIR piloting our core technology - this proposal outlines an initiative to validate & pilot a breakthrough of this platform, called IncludeConnect, that enables our technology to digitize with existing (non-smart)third party equipment and integrate it into our platform. With the core focus of creating next generation fitness & rehab protocols for our warfighters, specific applications within the DoD include data-driven, evidence-based return-to-duty physical health assessments, delivered autonomously, while providing best in class care for our warfighter post duty. Deployed across multiple sectors for care, it is our intent that any technology developed through this initiative then be deployed to our existing commercial customers in neuroscience, neurorehabilitation, orthopedics, pediatrics and the VA.
