It was launched in 2017 as healthcare specific venture capital fund of Leerink Transformation Partners by Todd Cozzens and Jared Kesselheim, MD. It makes capital investments in the healthcare technology and service industries.

The initial fund launched in 2017 closed with a value of $313 million. In November 2019, it was announced that the fund would establish its own firm, Transformation Capital, to operate the $313 million fund it has raised with . This followed the merger of SVB and Leerink Transformation Partners. The new firm would take an even sharper focus on its healthcare investing to concentrate on digital healthcare technologies.
